We accept payment by Visa, MasterCard, Discover, American Express, HSA cards, check or cash.
- Single Visit Treatment: Payment is due one the day of the visit
- Multiple Visit Treatment: 50% of the total treatment fee is due on the day treatment begins. The remaining 50% is due on the day the laboratory phase of treatment begins.
- Consultation fees: (Includes diagnosis and discussion of treatment options)
- General consultation fee is $77
- TMJ consultation fee is $120
Insurance Information
Since insurance is not designed to pay toward complex Prosthodontics:
- Our office does not participate in any insurance plans
- Our office does not accept any insurance payments
- We will submit electronic claims to your insurance 1x as a professional courtesy
- If your insurance does not except electronic claims you must submit you own insurance claim
- Patients must respond to all insurance company requests
- You may obtain your insurance claim forms from your insurance company
- We will provide you with the dental insurance codes for your procedures
